Breastfeeding/ Chestfeeding Initial Evaluation (2 hours) is $400 parent + baby

• - Comprehensive review of relevant aspects of yours and baby's medical history (including prenatal, birth events, postpartum)
  - Assessment of milk supply
  - Weighted feeds ( pre-& post-feed)
  - Full assessment of breastfeeding session (positioning, baby's latch, etc.)
  - Breast exam and flange size selection where you are fit and able to pump with your ideal size
  - Milk storage guidance
  - Oral sensory motor skills and full body milestones assessment

  - Guidance on bottle feeding, if applicable

  - Detailed care plan will be provided after the session with the option for a report to be sent to your pediatrician

• All follow up sessions (1 hour) are $200

Home visits do include an add-on travel fee that varies depending on driving mileage zone. Of note, travel fees are NOT reimbursable by insurance.

Therapeutic Bottle Feeding Initial Evaluation (1.5 hours) is $350 baby only

• The session focuses solely on baby and includes:
  - A detailed review of your baby's medical history
  - Assessment of oral sensory motor skills and full body skills
  - Assessment of bottle feeding session
  - Examination of any bottle-refusing behavior, if applicable
  - A detailed care plan provided after the session

Home visits do include an add-on travel fee that varies depending on driving mileage zone. Of note, travel fees are NOT reimbursable by insurance.

There are 3 parking lots to park in and the entrance is across from the CVS (which is also the back entrance to the Stop & Shop) and next door to a Carvel and Laundromat.

• Pumping Consult (1 hour) $200 parent only **Recommended for return to work prep**

• Prenatal Prep Consult (75 minutes) is $215 virtual only

• All follow up sessions (1 hour) to evaluations are $200

I do NOT accept insurance nor am I in network with any insurance carriers at this time. My practice does offer various tiers of support and packages depending on what best suits your family's needs.

1) Use of HSA/FSA cards

2) Superbill (billed under parent or baby) to submit to insurance

3) Buy Now / Pay Later options to make things easier financially (Superbill is available with this option as well) Bulk discounts of 3 or more visits included in this option

• SUPERBILL At your request, what we can provide for you at the conclusion of our session(s) is a Superbill that you are able to submit to your respective insurance company for reimbursement. Some families choose to reach out to their insurance company to receive pre-authorization of our visits- either as lactation, feeding therapy, or occupational therapy. We’d be happy to provide additional information upon request.

• Others have requested an occupational therapy prescription request from their infant’s pediatrician which they then submit to insurance to help increase the likelihood of reimbursement. At this time, we offer a complimentary OT prescription form faxed to your pediatrician’s office in an effort to maximize chance of coverage

Under the Affordable Care Act, insurance companies are mandated to cover lactation support. For more information, please visit the National Women’s Law Center . For WIC, Medicaid, or families experiencing financial hardship, please contact us.

Our payment processing system (Stripe) has three payment options are described below:

• Klarna is a buy now pay later method that enables customers to pay now, later, or in installments, depending on the country.

• Afterpay Clearpay is a buy now pay later method that enables customers to pay in 3 or 4 installments, depending on the country.

• Affirm is a 'Buy Now, Pay Later' method that enables customers in the US and Canada to pay in installments / monthly payments."

How it works:

1. You decide service type and session amounts you’d like the payments to cover. Typically, a bulk purchase of sessions (i.e. 1 initial evaluation, 4 follow ups) is recommended. Most of our families find an average 4-6 sessions most beneficial.

2. We’d send you an invoice for the session(s) and you’d choose which buy now / pay later service to use.

3. By creating an account with one of the three options, you’d accept or decline the terms of repayment and complete the necessary information/forms. Once the invoice is completed, we would then book your session and send you our intake forms.

If you decide to take advantage of the Buy Now / Pay Later option, we are still able to provide Superbills from sessions to submit to insurance for reimbursement!
